A lot of it is random idiots who don't spay and neuter their dogs rather than planned mixes.
They actually do ship dogs around the country. Usually from southern states. There are even some pet rescues involved in international adoptions.

Organizations do ship dogs, but you won't find many who will knowingly take in a dog with a bite history and not immediately euthanize it. Maybe a few, but it's rare.

I looked it up, and there have been some cases where people successfully sued the shelter after getting bitten by a dog somebody else got at a shelter. There might be some cases where someone got mauled by a dog they themselves got from the shelter. I didn't spend a lot of time looking into it.
If the shelter lied about the breed, or omits important information such as the dog having previous assaults, behavioral problems, illnesses, or not being fixed, you can sue them. But if you adopt a pitbull that says "Princess needs to be rehomed to a home with no pets because she is always fighting with the other doggies!", the shelter can just wash its hands.
