As far as I'm aware, it does, even in humans, unless you get specific anti-parasitic medication.
The oocysts aren't always immediately present in cat feces, I don't know if there is a specific documented trigger, but cats can definitely spread the disease way after initial infection on occasion. Outdoor cats don't normally live for very long anyway so it probably isn't super easy to document.
The parasite literally can not exist without cats. It's incapable of reproducing without cats, and the cats need to be roaming around outside to get it into the food supply.
